I did apply last year but moved from MD to DE in the middle of the process and was unable to submit the additional paperwork DE requested in time. Below is my list of health issues. Massive MI with CABAGE 1/2008 Back Problems beginning in 2008 Depression (2001-2002, 2007/8-present with increased meds) Pain intermittently from 2008 to 2012 Chronic Pain (average of 7) since 2012 Diabetic (2000) Arthritis discovered via blood tests and x-rays in 2014 in almost all my joints (neck, shoulder, spine, lower back, hips, & ankles) Have positive ANA with high white blood counts and platelets not diagnosed with Rheumatoid Arthritis but is suspected Allergies since child Adult Onset Asthma Polycystic Ovary Syndrome(1986) Bilateral Carpel Tunnel(2013/14) Dermoid Cyst Left Ovary (found on ultrasound 2014-have to change OB/GYN) Bilateral plantar fasciitis (1999-present. keep re-injuring it last time was found on xray in 2014) Prolactin Secreting Adenoma (2002 on MRI last MRI in 2007 did not find) Reoccurring right elbow lateral epicondylitis (1999-present) reoccurring right hip bursitis (1999-present) tendinitis of right ankle (1999) High Cholesterol (2001) Blood Pressure Meds prescribed to protect kidneys as am diabetic with slightly high blood pressure (2001) IBS (1999) Reoccurring Ganglion Cyst (1996-2005)

On 9/29/12 I was rear-ended at a red light. At the time of the accident I was unemployed, due to the fact my lower back and neck were swollen. I immediately went ER in Philadelphia on 10/29/12 due to constant pain. My lawyer Lundy Law, took my case an had me in therapy on 10/2/12. The pain still persisted and an MRI WAS taken showing a disc herniation coupled with a bulging disc with signs of spinal stenosis. Conditions worsened to go to an Orthopedic doctor which suggested injections a referred a pain specialist. My condition worsen until I had to rely on parents and friends to perform daily tasks washing cooking children activities etc. I went to another dr in Delaware which recommended injections They did not help I went to a neurologist on my own after two visits to ER AND SURGERY WAS scheduled for a microdisectomy IMMEDIATELY IN Jan 20, 2014. I have had improvent except got right foot toes. I fell down my stair due to the fact I could not feel them after numerous complaints. I fell down a flight of stairs and injures left shoulder and bruised back. I unable to work now and I the near future. My attorney is Joel Greenburg. I filed in 2013 I was denied due non-sufficient paper work. I completely files 5/14/2014.

I was on disability from August 2009 until March 2011. I went back to work on their ticket to work program on August 30, 2010. I was planning to stop working at the end of June 2011, since that would have been the 9 months I am allowed to work. After presenting my paystubs as was instructed with the program, I got a letter from the disability office claiming I owe all the funds given to me from August '09 until March 2011 (+40K). The letter that I received said why I owed back the money, claiming it is because I went back to work in Feb '09. I don't see how that is an issue if I came into their office for the first time Aug 11, 2009. Also to mention, I did not know I was going to receive benefits at all until January 2010. I was out of work August 09' and returned to work in October 09', because I hadn't heard anytthing from their office. Please help me or point to where I should go to get counsel.

I have severe inner ear problems that affect my balance and natural way of life. My doctor told me if I apply for assistance, he would let them know the situation. While going through the motions of applying for SSI, I was seeing another doctor (Infectious Disease Doctor) I was sent to by my primary ear doctor and he put me on an intravenous antibiotic, and I recieved a denial letter because a doctor that I've never seen that works for SSI said that the antibiotic would work... 2 days before the doctor told me he did all he could, but the treatment didn't work. This has been going on for years, but got much worse over the last few years. I've had 2 surgeries in the last couple years, and I continue to be dizzy constantly.

I have Rheumatoid Arthritis, Psoriatic Arthritis, Fibromyalgia, and am a single mom with three children. I have been trying desperately to keep working and find that the medications that keep my body from attacking itself make it really easy for me to get sick. I've attempted a career change also and that hasn't had a positive outcome. I'm working as a substitute teacher right now, which pays $83/full day of work, and we are struggling to stay alive because I find that working more than 2-3 days in a week leaves me too worn out for anything related to my children. I want to be able to work, and am finally facing the fact that working is making my health worse and that I really need to find a way to take care of my children that won't result in being fatigued every weekend because I overworked myself during the week or getting sick and not having the time to rest to get better because there is no income. Please help me, I don't know what to do or where to start!
